General Appearance Assessment

1. Apparent State of Health

Healthy/Well-appearing: Looks robust, energetic, comfortable.

Acutely Ill: Appears suddenly unwell, in distress, often with signs of pain or difficulty breathing.

Chronically Ill: Frail, fatigued, signs of long-term illness.

Frail: General weakness, muscle loss, slow movements.

2. Level of Consciousness and Orientation

Assess how awake and aware the patient is:

Orientation: Person, Place, Time.

3. Signs of Distress
4. Body Habitus and Nutritional Status
5. Posture, Gait, and Motor Activity
6. Hygiene and Grooming
7. Skin Color and Lesions
8. Facial Expression and Affect
9. Speech
